Wishes–Your end-of-life treatment preferences: Living Wills, Proxy Directives, and Physician’s Orders

This part of the website includes general and specific wishes; that is, your end-of-life treatment preferences.
Most people will want them to be kept confidential. but I have posted two forms which, in real life, are on opposite sides of the same piece of paper:
One side is the Advance Directive to Permit Natural Dying.
The other side is Physician’s Orders to Permit Natural Dying.

[1] Usable versions of the AD-PND and PO-PND forms are available only from Planning Professionals and physicians. That is why both pages have a watermark with the words, “Evaluation Copy.” Completing any Advance Directive requires diligence, however this is especially so with the AD-PND/PO-PND. These forms represent a person’s final orders and determine the choices for the kinds of treatments that have in the recent past been most contentious, prone to prolonged litigation, and affect loved ones deeply. Moreover, the forms are but one part of an integrated strategic program that includes keeping the forms secure so they will not be implemented too early. Since implementation of these forms mean the patient is ready to die, they must be completed and distributed with upmost care. This web site can be a place for us to share our fondest memories of a most wonderful, generous, loving, and philosophically inspiring gentleman: Harry Tillman.

Beth took the photo above in the summer of 2005.

Since Stan had to return to San Diego, he could not be present at the funeral, however his words, said to Sylvia, Suzan, and Jon, and his cousins Bonnie and Bill — were recorded on April 16, 2007. Click here to listen to Stan’s (less than seven-minute long) eulogy.

If you are interested in a more detailed description of Harry’s last few days, click here. (Note: While this IS Harry’s story, since it is published on a public web site, I changed the names to “Henry” and “Sarah.”)
